In 2025 Round 2, there will be a pilot of evidence pro formas, which must be completed and uploaded alongside any evidence submitted for the following domains:  Presentations and posters, Publications, Teaching experience and Quality improvement. Forms can be downloaded from the Document Library.

The forms have been introduced following feedback from assessors and are intended to help make your achievements, and the role you played, as clear as possible to assist the verification process. There is a degree of overlap between the information requested on the pro formas and application forms, and it is fine for applicants to copy information between the forms where relevant. 

This pilot process will be assessed after shortlisting is completed in September/October 2025, and a decision made about their continuing usage in 2026.
